Thank you for your interest in this study. This study plans to learn more about the ways that environmental factors such as stress and stressful events affect the physical and genetic characteristics of human sperm. Before you learn more about the study, it would be helpful to see if you are likely to qualify to be in the study. In order to do this, please complete this survey which will ask you some eligibility questions, including questions about yourself and your health history. It should take about 5 minutes to go through these questions.
Some of the questions may make you uncomfortable; you do not have to answer any question that you would not like to answer, but without answers to these questions, you will not be eligible to participate in the study. The researchers will not record your name or any other information that would identify you based on this survey until you are deemed qualified to participate in the study; at that time, the researchers will keep this information secure.
If you do not enroll in this study, the researchers will keep the information collected during this pre-screening but there will be no way for anyone to link that information to you.
