Why is this study being done? 

 

Thank you for your interest in this study. This study plans to learn about whether receiving pre-exposure prophylaxis (PrEP) through telemedicine for HIV prevention works for youth and young adults. Telemedicine involves having your appointment virtually by phone, tablet/iPad, computer, etc. Instead of in person at the clinic. If you are eligible and agree to participate in this study, you will be referred to a clinic provider, do several laboratory tests as clinically indicated, and be provided with a prescription for daily oral PrEP medication.  

 

Why are you completing this questionnaire? 

 

This form will ask you some questions to find out more about if you might be eligible to participate in this study. It should take about 5 minutes to go through these questions. Some of the questions may make you uncomfortable. You do not have to answer any questions that you would not like to answer, but without answers to these questions, you will not be eligible to participate in the study. If you have qualified for the study, you will be able to record your name and contact information so our study team can contact you about participation.  

The data we collect will be used for this study but may also be important for future research. Your data may be used for future research or distributed to other researchers for future study without additional consent if information that identifies you is removed from the data. 

We are also required to give you the number of COMIRB, the Ethics Board that oversees our research: it is (303) 724-1055, in case you have any questions or concerns for them.
