You are being asked to participate in this research study because you are a healthcare provider in the state of Colorado. Your input is highly valued and greatly appreciated.
If you choose to participate in the study, you will complete a confidential survey. Every effort will be made to protect your privacy and confidentiality. No personal identifying information will be collected. Your survey will not be linked to you by e-mail address or IP address. It should take no more than 20 minutes to complete. You will only need to complete the survey once and no additional follow-up will be asked of you.
The study is designed to learn more about provider knowledge, attitudes, and behaviors regarding patient education on the safe use of opioids.
Possible discomforts or risks include time required to complete the survey and the risk for breach of confidentiality. There may be risks the researchers have not thought of.
You have a choice about being in this study. You do not have to be in this study if you do not want to be.
