Applied Outbreak Investigation Pre-Training Evaluation

Please complete the survey below.

As part of the Applied Outbreak Investigation Training, we are asking participants to complete 3 short (~5 minute) training evaluations. The information collected from these evaluations will be used to improve the training and guide future training development, which will help the Food Safety Centers of Excellence continue offering trainings for free. You will be asked to complete 3 surveys at different times over the course of the training:

A pre-training survey before the training
A post-training survey immediately at the end of the training
A follow-up survey 6 months after the training

Each survey is optional and responses will be anonymous and analyzed collectively with those of other participants.

A Certificate of Completion will be emailed to you after you complete the post-training survey.
